eStatement Enrollment Request

  • I have established online banking credentials with Leader and am able to access online banking for all Accounts through those credentials.
  • I can access all documents and statements within Leader’s online banking system and have access to AdobeAcrobat Reader 5.0 or above or a file reader otherwise capable of accessing .pdf extension files.
  • I authorize Leader Bank to provide any periodic statement generated for my Accounts using eStatements, as well as any other periodic or special notices which may also be sent by email (including availability of funds, error resolutions notices, privacy notices, etc.).
  • I understand that I will not receive statements for my accounts by mail effective upon the activation of my eStatements.  Rather, I will receive notification at this email address each time Leader prepares a statement for the Accounts notifying me that the statement is online.  It shall be my responsibility to log into online banking to view the statement.
  • I understand that Leader Bank has no control as to any person who has access to my computer, device or online banking credentials and I shall be liable for any unauthorized access to such computer or device or use of my credentials.’
  • I shall beresponsible for notifying Leader of any change to the email address associated with my Accounts or online banking profile by email, phone or in person.  Leader shall have no obligation or liability to any party to a joint or multi-party account if the email address is changed pursuant by an authorized owner or signer on the account.
  • I understand that all eStatements shall be dated the day I receive notice by email of the availability of said document.  I agree to promptly review all eStatements and accompanying documents and notify Leader promptly in writing of any error or unauthorized transaction on any Accounts.  The applicable time periods in which I must notify Leader of any error on any periodic statement shall begin on the date I receive notice by email of the availability of the eStatement regardless of whether I access the actual eStatement on that date.
  • I understand that I have the right to withdraw my consent to receive electronic periodic statements atany time by contacting Leader Bank by email, through online banking, by telephone or in person, and that it may take up thirty (30) days for Leader to update my account to resume paper statements.
